  • This form must be filled out in its entirety and returned to Solutions by Text before the 10DLC provisioning process can begin.
  • All fields are REQUIRED unless otherwise noted. Incomplete or inaccurate information may result in a delay.
  • Billing for the 10DLC and any relevant campaigns begins the date the application is submitted, before the number is registered for use and/or live with all wireless carriers.
  • Wireless carriers may require additional information after the registration process begins.
